- Name and billing address
- Service facility location
- Telephone & fax numbers
- National Provider Identifier (NPI)
- Tax identification number
- Date of birth
- Patient billing information for the claims processing and patient bills
- We will use the provided data to process claims, submit patient bills, determine patients eligibility for coverage, obtain authorizations, respond to insurance carrier requests or respond to patient inquiries only.
- Each physician or therapist has a separate database with private access codes and strict privacy safeguards for your data. We review these safeguards regularly with our computer technician for new technological advancements. We also train our employees in the proper handling of your database.
- All correspondence via fax and email is also privacy safeguarded.
